At Golden Valley Memorial Healthcare, our primary care providers offer a patient-centered medical home — an approach that emphasizes comprehensive, proactive and preventive care designed to promote overall wellness. Unlike the traditional reactive model that addresses health concerns only when you or your family are sick or injured, we focus on keeping you well while still providing care when illness or injury occurs.

As a patient-centered medical home, we prioritize building strong, long-term healing relationships between you and your care team. Together, we serve as your central partner for all healthcare needs, working collaboratively to help you and your family maintain the best possible health.

In this model, patients and providers work closely in partnership. While we take responsibility for many aspects of your care, we also encourage you to actively participate as a vital member of your care team.

  • Communicate treatment goals and options clearly
  • Develop treatment plans collaboratively

  • Offer information about classes, support groups and other resources

Your Responsibilities to Yourself and Your Care Team:

  • Actively engage by asking questions and sharing your needs
  • Provide a complete and accurate medical history
  • Inform us about any care you receive elsewhere, such as hospital admissions or emergency visits
  • Follow your agreed-upon care plan
  • Participate in self-care activities
  • Share feedback about your health and wellness

Your Dedicated Care Team

As your patient-centered medical home care team, we are your main point of contact for a wide range of health needs. We assist with planning, scheduling and coordinating care across various settings and specialties. Specifically, we help you:

  • Schedule and complete necessary labs or tests, keeping all providers informed
  • Collaborate with specialists to manage your care
  • Proactively arrange preventive screenings and exams
  • Follow up on hospital or emergency room visits, scheduling additional care as needed
  • Provide support from social workers, dietitians and other care team members
  • Offer education and connect you with community resources to help you manage your care
  • Provide long-term management for complex or chronic conditions, coordinating involved providers

Contact Your Patient-Centered Medical Home

For your convenience, we offer multiple ways to connect with us and access your medical information:

  • Visit us at one of our Clinics, Walk-in Clinics, or via telehealth
  • Reach us 24/7 through after-hours on-call providers
  • Access your medical information through the myGVMH Portal
  • Message your provider directly via the myGVMH Portal

If you are discharged from the GVMH Hospital or our Emergency Department, your care team nurse is available by phone to:

  • Review medications
  • Discuss discharge instructions
  • Schedule follow-up appointments with your primary care provider
  • Provide patient education materials
  • Offer referrals to local resources for food, housing, and safety, if needed
