At Golden Valley Memorial Healthcare, we understand that your health and well-being depend on more than just medical care. That is why we are proud to be a part of ToRCHLink, a collaborative group of rural hospitals working to implement the Transformation of Rural Community Health (ToRCH) model in Missouri.

Our mission is to support Medicaid recipients in Henry County by connecting them with essential services that can improve their everyday lives. From transportation to meals, home safety improvements and more.

ToRCHLink helps reduce barriers to care and improve quality of life. Whether you need help getting to a doctor’s appointment, guidance on nutrition, home-delivered meals or assistance making your home safer and more accessible, ToRCHLink is here to support you.

To qualify for ToRCHLink services, you must:

• Be a resident of Henry County
• Have active Missouri Medicaid (including Managed Care and Dual plans)

Note: If you have a Medicaid spend down, it must be met for the month in which you are requesting services.

Transportation Support

ToRCHLink helps cover transportation costs through our partner, New Growth Transit.

• Receive $20 per one-way trip, plus $1.25 per mile

Nutrition Counseling

Work with experts to make healthier food choices with support from Diatribe KC and MU Extension.

• Receive up to $25 per year in personalized nutrition counseling

Home-Delivered Meals or Groceries

Meals are provided by Clinton Christian Church and are available for up to three meals per day, for up to 12 weeks (renewals possible).

• Cost: $9.44 per meal
• Choose between ready-to-eat meals delivered to your home or groceries to prepare yourself

Environmental Modifications (Medical diagnosis required)

If a medical condition affects your mobility or safety at home, ToRCHLink can help make necessary changes.

• Up to $4,000 per year for home modifications like ramps, grab bars and more
• Trusted local partners include Helm Plumbing and Electric, Logan’s Heating and Cooling, Sherman’s Appliances and West Central

Goods and Services

In addition to home modifications, ToRCHLink can help cover the cost of essential home items and services.

• Up to $1,500 per year for items such as a refrigerator, stove, air conditioner, water heater or pest control
• Services provided by local partners, including Crain Pest Control, Helm Plumbing and Electric, Logan’s Heating and Cooling, Sherman’s Appliances and West Central
