The diabetes self-management education and support services (DSMES) at Golden Valley Memorial Healthcare is an American Diabetes Association (ADA) recognized program that provides an individualized education and management support system to empower patients with the knowledge and tools they need to manage their diabetes independently.
Our evidence-based curriculum from the International Diabetes Center (IDC) may include individual and group training and medical nutrition therapy. These courses require a provider referral. After the referral is received, we will call to schedule your appointment.
The initial visit usually lasts at least one hour. For your convenience, group classes may be scheduled as three two-hour classes or you may attend a six-hour class on a single day. DSMES costs may be covered by some insurance. Please check with your insurance provider for authorization prior to your first visit. Medicare coverage has been established for those with diabetes and renal failure. Your visits may be covered under your employer’s cafeteria plan. Visits may also be tax-deductible and financial assistance is available for those who qualify.

Our diabetes care team hosts free diabetes management support groups for people with diabetes and their support people. Each of the support groups are designed to create an opportunity for people with diabetes to connect, ask questions and make new friends.
