Home Health

Our home health agency provides assistance to you or your loved one during recovery from an illness, surgery, injury or long-term medical problem in the privacy, comfort and security of your home. Skilled nursing, behavioral health services and therapy (physical, occupational, speech) services are delivered in the home with the goals of improving health, regaining independence, becoming as self-sufficient as possible and/or maintaining the current condition or level of function.

The home health program intends to improve quality of life and increase the patient’s strength, endurance and functional abilities. To qualify for home health services, a patient must meet the following criteria:

  • Be considered “homebound” based on the Centers for Medicare and Medicaid guidelines
  • Require skilled care on a part-time or intermittent basis to improve, maintain, prevent or further slow the health condition
  • Be under the care of a doctor, who completes and documents an in-person visit

Our home health professionals have been recognized for their exceptional commitment to the patient experience. In 2019, they were named a Top Agency of the 2019 HomeCare Elite®, a recognition of the top-performing home health agencies in the United States. The home health team also received a five-star rating, on a one-to-five scale with five stars being the best, from the Medicare.gov Home Health Consumer Assessment of Healthcare Providers and Systems, which measures the experiences of people receiving home health care from Medicare-certified home health agencies.
